Meal Voucher Program Application Form

The Meal Voucher Program provides eligible students with up to 3 meal vouchers per quarter. Vouchers may be redeemed for a free meal at any of the three dine-in dining commons at UCSB (Carrillo Dining Commons, De La Guerra Dining Commons, and Portola Dining Commons). Students are eligible if they have: 1) a self-identified need for food assistance, 2) current enrollment status at UCSB as an undergraduate or graduate student, 3) completed the A.S. Food Bank registration form since Jul 1, 2024, and 4) not yet received the maximum 3 meal vouchers for the current academic quarter. If you meet the eligibility criteria and are interested in applying for a meal voucher, please fill out this form. Note that each form submission is for 1 voucher (max. 3 vouchers per quarter). If you are interested in applying for multiple vouchers, please submit another form. Applications are reviewed on a first come, first served basis. Individuals will receive further instruction on how to pick up their issued meal vouchers at the A.S. Food Bank’s main pantry upon approval. Please note that vouchers may only be redeemed by the approved individual, and must be used by the expiration date stamped on the back of the voucher. Students must provide valid identification at time of redemption.
